Are you accepting new patients?
Yes. We are currently accepting new patients of all ages. Call us at (732) 494-4500 or use the appointment-request form to get started.
What should I bring to my first visit?
Please bring a photo ID, your insurance card, a list of current medications (including dosages), and any relevant medical records or test results from previous providers. Arriving 15 minutes early to complete paperwork is appreciated, or download new patient forms above to fill out at home.
How do I request a prescription refill?
The fastest way is through your pharmacy โ€” they'll send the refill request to us electronically. You can also message us through the patient portal. Please allow up to 48 hours (excluding weekends) for refill requests to be processed.
What do I do if I need care after hours?
For a true emergency, call 911 or go to your nearest emergency room. For urgent but non-emergency concerns when our office is closed, please use an urgent care center and let us know at your next visit so we can update your records.
How can I get a copy of my medical records?
Complete the Records Release form above and submit it to our office. Records can be sent directly to another provider or released to you. Please allow 7โ€“10 business days.
Do you offer telehealth visits?
For certain types of visits โ€” such as follow-ups for stable chronic conditions, medication checks, or some sick visits โ€” telehealth may be appropriate. Call our office to discuss whether telehealth is a fit for your concern.
